It is indeed a great privilege to be associated with the effort of the Journal of Clinical Epidemiology (JCE) to bring out an Indian edition. The selection of articles for this edition has been primarily from the clinical epidemiology units (CEUs) of the Indian Clinical Epidemiology Network (IndiaCLEN) and therefore does not truly reflect the diversity of research in the country. However, this issue of the journal reflects the pattern of clinical epidemiologic research carried out in India. I believe that each country brings its own priorities to the fore, and it would be interesting to study the response to research challenges faced by the clinical epidemiologists in different regions of the world.

Diarrhea is one of the principal causes of morbidity and mortality among children in the developing world. Cumulative costs of treating diarrhea would be high and would further increase if zinc was used as an adjunct to treatment of acute diarrhea.

Evidence-based medicine (EBM) is the integration of current best evidence with clinical experience and patients' values and preferences in clinical decision-making process [1]. Clinical experience has played a key role in clinical decision-making process since ages. But research evidence and patients' preferences have received a variable degree of attention in the process. With the introduction of EBM, the clinicians are exhorted to consider all the three factors in the process.

The present study evaluates the feasibility of rapid surveillance of community antimicrobial resistance (AMR) patterns of Streptococcus pneumoniae and Haemophilus influenzae in India using nasopharyngeal swabs (NPSs) of school children. It compares the AMR data obtained with that of invasive and nasopharyngeal (NP) isolates studied previously. No one has done such surveillance since our study so we decided to publish and more clearly demonstrate the feasibility of the methodology we did.
